We can help you navigate the aged care system through our free Care Finder service.
Care Finders support vulnerable older people to navigate the aged care system and find support services to improve your quality of life.
We operate in the ACT and parts of Southeastern New South Wales. Our Care Finders can meet with people face-to-face or online, in our offices or where they feel most comfortable.
Eligibility
Coordinated by the Australian Government’s Department of Health, Disability and Ageing, the Care Finder program is available for:
- people aged 65+
- Aboriginal and Torres Strait Islander people aged 50+
who require intensive support to access My Aged Care and navigate relevant supports within their community.
It is for people who are especially vulnerable due to circumstances such as, but not limited to:
- not having family, friends, carers or representatives they can receive support from
- being hesitant to engage with aged services or other organisations due to negative past experiences
- being homeless or at risk of homelessness.
Our Care Finder service is for all eligible older people; you don’t have to be part of Meridian’s core communities to access this service. However, we are aware that older LGBTQ+ people face unique challenges, barriers and vulnerabilities, and can help with these through the program.
Get involved
Anyone can refer an eligible recipient to our Care Finder service. You can be referred by:
- yourself
- family, friends or health services or aged services providers
- specialist aged care assessment services.
