WHAT INFORMATION WILL I NEED TO PROVIDE AT MY FIRST APPOINTMENT?

At your first appointment you will need to complete a Client Details Form including basic demographic details and information that will help us help you such as your pronouns, an emergency contact person and how you identify your gender (if at all).

We will also ask you to consent to Meridian holding your details in confidence and as per the Privacy Act for the purpose in which they were provided. Some data that does not identify you in any way is also requested.
